Most pregnant women, mid pandemic or not, will be asked to create a birth plan, a brief document outlining preferences on everything from the setting to pain management to medical interventions. But, as anyone who’s drawn up one for themselves knows, birth never really goes as planned.

For families who are currently expecting, the emergence of COVID-19 has heightened that feeling dramatically. Further compounding that is the announcement this week that two major New York City–area hospital groups ( and ) would be banning all support people—including spouses and partners—from labor, delivery, and postpartum units. An effort to limit potential additional exposure to the virus, the move would leave women to labor without a trusted person to rely on and advocate on their behalf.So what should pregnant women consider as they’re adjusting birth plans and taking precautions? Here, experts weigh in on what to know about pregnancy and the coronavirus. The Birthing Experience. CNM, a midwife at, advises that having a support person and advocate during birthing can significantly affect a woman’s experience and outcome.

“There’s a lot of good research that shows that having a support person with you in labor decreases rates of epidurals, cesarean sections, and many other interventions,” she says. Lubell, who delivered three babies at Mount Sinai West in the days before the announcement, says hospitals are doing everything they can to safeguard pregnant patients: Masks are worn 24/7, there are extensive handwashing and sanitizing protocols, and there’s extremely restricted access so there’s no unnecessary exposure. “I understand the desire to stay away from hospitals right now, but labor and delivery is a dedicated space, and those units are completely protected,” adds, an ob-gyn who practices at Lenox Hill Hospital, part of Northwell Health (where, at press time, one support person, who must remain masked, is still allowed with the patient in her labor room if they’re asymptomatic). But the uncertainty around COVID-19 has led some women to consider pivoting to a home birth. Sanghvi—who is against home births, pandemic or not—worries that if emergency situations arise and women have to be transferred last minute to a hospital where the staff is already overburdened, it could heighten risk factors for the patient.

Lubell believes fervently that the home is traditionally a safe place to give birth if you’re under the guidance of highly trained and experienced practitioners following guidelines set out by organizations like ACNM or MANA. But even she is concerned about the current rise in home-birth requests. “As someone who works in a hospital and had a home birth myself, I believe you need to have chosen a home birth for the sake of wanting to give birth at home,” she says. “Just being scared of COVID-19 is not a good reason.” She adds that home-birth midwives are often going back and forth between different residences and potentially have higher rates of exposure. Protecting Yourself While PregnantBecause the virus has only been active for about four months, there’s very limited data that can be validated. But from what experts have seen with cases in China and Italy, pregnant women don’t appear to be at an increased risk of contracting COVID-19, as is the case with the flu or other respiratory illnesses.

“Pregnancy itself is a state in which a women’s immune system is suppressed, so theoretically a pregnant woman may be more susceptible, but based on our limited experience with COVID-19, we haven’t seen that,” says Sanghvi. What we don’t yet know is what might happen if COVID-19 is contracted in early pregnancy. “All the information we have at this point is for women who developed COVID-19 in their third trimester,” says Sanghvi.

What is reassuring is that children seem to be protected: Unlike with the flu, which qualifies newborns and infants as a high-risk group (along with the elderly and pregnant), they are not more vulnerable. It’s also comforting that there’s zero evidence to date of vertical transmission (when a disease is passed from mother to fetus via the placenta). “Testing of amniotic fluid and umbilical cord blood and throat and nasal swabs of newborns born to parents positive for the virus do not show any signs of it,” Lubell adds. Because pregnant women aren’t at a heightened risk for acquiring COVID-19, they should employ the same precautionary measures as everyone else: and isolating as much as possible, frequent and thorough handwashing, not touching your face. If you start to run a fever, says Sanghvi, don’t panic; a fever can be an indication of several things (like a cold or flu) that aren’t COVID-19.

Her recommendation: Call your doctor for guidance, but don’t just show up to their office or an E.R. (Many hospitals, including, have pregnancy-specific hotlines set up to field questions.) And take a Tylenol. “Tylenol is the medication of choice to treat fevers in pregnancy, and taking a Tylenol to treat one is much safer compared to the potential risks of a high fever,” says Sanghvi. If you’re pregnant and think you may have been exposed, you may still be asked to stay home and monitor symptoms as access to tests remains limited.

Pre- and Postpartum SupportThe postpartum support network (doulas, lactation consultants, and yes, grandparents and family too) must, for the time being, be limited to online contact. Since doulas are no longer allowed in labor and delivery units, many of them have quickly pivoted to offer extensive virtual assistance, both during labor via FaceTime and in the months before and after. “I’ve spoken to a number of doulas who are all bumping up their prenatal and postpartum visits virtually with their clients and are offering them a lot of support even throughout their labor,” says Lubell. Doulas like (based in Kingston, New York) and the bicoastal team at are already fully virtually enabled. Lactation consultants are also shifting to a virtual model to assist women with any breastfeeding issues in those first months postpartum; offers a direct link to many virtual providers. The protocol for prenatal appointments has, of course, also changed: Most doctors and midwives are drastically scaling back in-person visits, extending the interval between them, and relying heavily on telemedicine.

At Sanghvi’s office, where just she and an assistant are working, appointments are significantly spaced out, patients are escorted directly into exam rooms, and social distancing and disinfecting measures are being taken. “There are some time-sensitive tests that need to be done, but it’s reasonable to skip certain ultrasounds,” she says.Pregnancy and have always been times of increased anxiety, so emotional-support networks are still vitally important.

“The emotional landscape is different for pregnant women and new mothers,” says Dr. Catherine Birndorf, a psychiatrist and medical director at, which she cofounded. “It’s Darwinian that women are already in a heightened state of vigilance where they’re hyper-protective of their fetus or newborn, and COVID-19 is throwing their anxiety into overdrive.” The Motherhood Center has started offering weekly webinars to provide some coping tools for these mounting anxieties and has shifted its mental health services for new and expectant moms to a telemodel. Getting PregnantFor those who are trying to conceive, all the uncertainty around COVID-19 can feel especially unnerving.

It’s not a bad idea to consider reaching out to a mental health professional for some guidance. (The Motherhood Center is doing telemental health visits). Ultimately, though, the decision comes down to a personal choice. “There are some couples who are incredibly risk averse who will say, ‘Let’s just wait six months until we know more,’ and there are others who, perhaps because they’ve been trying for a while or are older, are not going to want to wait,” says Sanghvi. Do consider that, when it comes to resources, many health care providers and hospitals are stretched thin.

But if personal safety is your main concern, from what we know now, as long as you follow the guidelines to protect yourself from the virus, you should be okay. As Lubell notes, “Hopefully people becoming pregnant now, by the time they give birth we will be done with all of this.” Abortion AccessAnother concern for women amid COVID-19 may be: Will I be able to terminate a pregnancy, since abortion is considered an elective procedure? While other states are trying to restrict access, Sanghvi says that in New York you will be able to get an abortion, just not in a hospital setting. “There are doctors who do perform elective abortions in their offices, and outpatient abortion clinics staffed with board-certified obstetricians are still able to see patients,” she says.

And if it’s early enough, access to medical termination (using medication such as Plan B to end a pregnancy) is also an option.
